Does Cannula Material Influence Phlebitis Risk in Intensive Care?

Study Overview
Short peripheral cannulas are commonly used in intensive care, but mechanical irritation from the catheter may contribute to phlebitis. Although polyurethane is generally considered a flexible catheter material, different polyurethane products may vary in their composition and stiffness.
This post-hoc analysis of the multicentre AMOR-VENUS study examined whether phlebitis rates differed according to the material and product used. The analysis included 1,355 adult ICU patients and 3,429 short peripheral cannulas across 23 ICUs in Japan.
Key Findings
Phlebitis rates varied between cannula products, including between different types of polyurethane cannulas.
The highest incidence was observed with the SuperCath® polyurethane cannula, at 13.1%.
Compared with other polyurethane cannulas, SuperCath® and ethylene tetrafluoroethylene cannulas were associated with a higher risk of phlebitis after adjustment for other factors.
PEU-Vialon® cannulas did not show a statistically significant difference compared with the reference polyurethane group.
These results suggest that broad material categories alone may not fully explain catheter performance. Differences between individual products, including their physical properties, may also be important.
Implications
Cannula selection may influence the risk of phlebitis in critically ill patients, but the findings should be interpreted carefully. This was a secondary analysis of observational data, so it identifies associations rather than confirming that a particular cannula product directly caused phlebitis.
The study highlights the need to evaluate individual catheter products rather than assuming all polyurethane cannulas perform in the same way. Further prospective research is needed to examine characteristics such as catheter stiffness, manufacturing processes, insertion practices and patient-related factors.
For clinical services, the findings support monitoring phlebitis outcomes alongside the specific cannula products used locally.
